3. Fill out the reservation form below and submit.
   

That's it, you will immediately receive a confirmation email from me that your submission was received and placed for review. Upon review and approval your chosen date will be entered into the calendar.

Connie Wollenhaupt reserves the right to accept or reject any submission. Should your request be rejected, your deposit will immediately be refunded.

This is a Contract for services of Artist(s), made on date of form submission between the Purchaser and Artist(s), representing an Agreement to the following conditions:
